Good Morning Health is committed to protecting the privacy of your health information. This Notice describes how we may use and disclose your protected health information (PHI) to carry out treatment, payment, and health care operations, and your rights regarding that information.

How we may use and disclose your information

We may use and share your health information for treatment (coordinating your care with specialists or pharmacies), payment (submitting claims to your insurer), and routine health care operations (quality improvement and practice management). We may also contact you about appointment reminders, treatment alternatives, and health-related benefits.

Uses requiring your authorization

Any use or disclosure of your health information not described in this Notice will be made only with your written authorization. You may revoke that authorization in writing at any time, except to the extent we have already acted on it.

Your rights

You have the right to: inspect and obtain a copy of your records; request an amendment if you believe records are inaccurate; receive an accounting of certain disclosures; request restrictions on certain uses; request confidential communications; and obtain a paper copy of this Notice.

Our responsibilities

We are required by law to maintain the privacy and security of your health information, to notify you following a breach of unsecured PHI, and to abide by the terms of this Notice. We may update this Notice as required, and the revised version will apply to all information we maintain.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us and with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
