A little preparation goes a long way. Here's what to bring and what to expect — so your visit is focused on care, not paperwork.

What to bring

  • Photo ID (driver's license or state ID)
  • Insurance card(s)
  • A current list of medications, supplements, and dosages
  • Any relevant imaging, labs, or records from other providers
  • Your copayment, if applicable
  • A list of questions or concerns you'd like to discuss

Before you arrive

Arrive 10–15 minutes early to complete any remaining intake forms. If you're a new patient, complete your registration ahead of time through the patient portal to save your visit time for the conversation that matters.

During your visit

We start by listening. Expect time to explain your history and goals, a clear assessment, and a plan you understand before you leave. Bring your questions — there's no such thing as too many.

After your visit

You'll leave with a written summary and next steps. Follow-up instructions, referrals, and results are posted to your patient portal, and our team follows through rather than leaving you to chase answers.

Telemedicine visits

Joining a virtual visit? Find a quiet, private, well-lit space and test your camera and microphone a few minutes early. Have your medication list and questions handy, just as you would in person. We'll send a secure link before your appointment.
